As a pivotal policy dialogue on enhancing public health responses to substance use concludes, participants have expressed a strong commitment to addressing this pressing issue. Over the past three days, experts and stakeholders have engaged in meaningful discussions, culminating in a consensus statement that emphasizes the need for a comprehensive approach to tackle addiction as a significant public health challenge rather than a law enforcement issue. The dialogue highlighted the alarming treatment gap of over 92% for individuals with substance use disorders in the Eastern Mediterranean region.
Recognizing Substance Use as a Public Health Crisis
The dialogue underscored the importance of shifting the perception of substance use from a criminal justice concern to a public health priority. Stigma and discrimination against individuals with substance use disorders have created barriers to treatment and support. Participants emphasized that strong leadership is essential at various levels, including government and public health sectors, to implement coordinated and evidence-based interventions. The need for robust policy and regulatory frameworks was identified as a critical first step in addressing this challenge.
Moreover, enhancing health systems to improve access to care for those affected by substance use disorders is vital. This includes integrating substance use services into national health coverage, which would ensure sustainable resources and expand access to prevention, harm reduction, treatment, recovery, and rehabilitation services. The dialogue participants called for a comprehensive approach that includes improving health literacy and establishing national prevention systems that foster collaboration across different sectors.
Strengthening the Health Workforce and Education
To effectively combat substance use, it is crucial to equip the health and social care workforce with the necessary skills and knowledge. This can be achieved by integrating substance use education into undergraduate and in-service training programs for health professionals. Additionally, the establishment of postgraduate addiction medicine programs will further enhance the capabilities of the workforce. Ensuring the availability of essential medications while preventing non-medical use is also a priority.
The commitment from UN agencies to provide technical support for these initiatives was reiterated, with a focus on minimizing harm, improving treatment accessibility, and promoting recovery and reintegration into society. The establishment of a Strategic and Technical Advisory Group (STAG) on Substance Use was announced, which will guide countries in developing evidence-based policies and services supported by robust monitoring and evaluation mechanisms.
Launching Collaborative Efforts and Regional Coalitions
In a significant move to foster collaboration, the Regional Mental Health and Substance Use Coalition was launched, bringing together civil society organizations dedicated to mental health and substance use issues. This coalition aims to enhance literacy around substance use and combat stigma by empowering individuals with lived experiences. An interactive platform, developed in collaboration with the Global Mental Health Action Network, is set to launch in April, ahead of the 6th Global Ministerial Mental Health Summit in Doha.
Participants expressed a strong desire for support in initiating national dialogues on substance use and establishing governance structures for national programs. The World Health Organization (WHO) pledged to assist in developing balanced national action plans, reviewing legislation, and strengthening harm reduction and treatment services. The commitment to sharing best practices and forging partnerships with centers of excellence in other regions was also emphasized, aiming to create a network of support and knowledge exchange.
